Basically people just create a photoshop template and pass it around. Then you just find high quality pics of your favorite players and either make your own or have someone who is experienced with photoshop to do it for you.

Personally I don't care too much if the cover has aesthetic appeal or not, once I start playing the game I would never even consider it. I have to admit though, this cover is just atrocious.
Madden NFL 2006 will be out in a matter of months, so it's that time again. The man to grace the cover of this years Madden game, and likely have a horrible season, is none other than Donovan McNabb of the Philadelphia Eagles.
