I'd put my money on it. The shading on the thumb is off, there's more light then there should be which leads me to believe that the original pic was with his thumb over the eye/eyebrow and is was just erased and replaced over. For it to be in the eye socket, it'd be shaded a little more, that's why I think.

I've seen it before and I don't believe it is 'shopped'. I think the thumb is actually bent in slightly, so it's not as deep as it appears in the socket.

I think you hit the nail on the head. His thumb could be bent but it appears in such a fashion that it looks like it's impailing his eye socket
