Help on a QB Avatar Tavaris Jackson

Hey Im looking for some help on an Avatar that I will be using on all my football related forums, such as betting forums and fantasy football forums. This needs to be an avatar and not a wide signiture, so this should be fairly easy for someone to make.

I have the picture that I want and I was hoping you could crop out the background and then put the name somewhere convenient that will be big enough to see as an avatar. It doesn't have to be a specific size because it will automaticly resize down. The size will be aproxx the size of the add on the left hand side of this site, The "get ready the 2006 season kicks off."

I tried it myself in photoshop, but I had trouble with the helmet. Hopefully I can get some help. thanks!
