I'm in the fifth year of running my league, which has always been a straight-up Yahoo 10 team league of buddies and coworkers.
This year, I'm changing it up, going to fleaflicker or MFL, adding a flex, and expanding the league (I had to turn away several last year).
How big is too big in your opinion? 12 teams seems a good size, but is 14 pushing it?

I did this this year. I went from 10 teams to 12 teams, but also increased amount of teams that make it to the playoffs. Instead of 4 there are 8 and an extra week of playoffs. I also made a 4th money spot. I believe you just need to keep everything proportional.
I like 12 team leagues. Seems to have a very good balance. I have nothing against 14 team leagues either, the more managers the better you have to be to win.
However, I wouldn't say if you have 12 teams that 8 should make the playoffs. That is kind of rediculous. Chances are that 2 of those teams are under 500... And they should not be in the playoffs.
When there are 12 teams, I like a 3 week playoff system with 3 divisions. 3 division champs, 3 wild cards. Top two division champs get a bye the first week. Rewards your best teams.
I've got a 14 teamer set, with the top 6 going to the playoffs.

Agreed. Plus, sitting through a draft with 14 teams and more than 15 rounds would SUCK!
I've also added an IR spot because rosters will be so thin.

I'm having the same issue here except my league is in it's first year and I already have 12 guys locked into a 12 team league but have 2 more wanting in. So from reading this I think I'm going to do the 2 team expansion to 14 but reduce the roster size to 14. 8 starters and 6 bench players while setting max limits on qb's, rb's and wr's to make it more competitive and less likely one owner can stockpile the entire afc all pro team from last season. i currently have my playoffs set at 6 top 3 from each conference. does this sound like it will work well?

I'd do away with position limits. If somebody wants to burn 5 roster spots on QB's, why not let them.
The starter/bench set up seems fine.
IMO, it is fine to take just 4 teams to the playoffs in a 14 team league. You can take 6 if you want, I guess
You mention two conferences. Are you just doing 2 conferences of 7 ?
