In the case of "ground caused the fumble" I think they are actually reviewing "down by contact". There are certain fumbles that can be reviewed because there are exceptions but generally speaking fumbles cannot be reviewed.

It doesn't matter if he was touched or not. He never had possession so it was a live ball when Smith pitched it and then it is just one long continuous fumble that cannot be reviewed.

Fumbles are almost always reviewed unless it's obvious. It probably wasn't reviewable for some other reason. Cotch was laying on the ball and was touched prior to the ball squirting out. Jets ball!
