Title bar request

A little different request than normal. I am doing a weekly MVP article for my bar and was trying to locate a title bar just like the ones you see on ESPN or FOX during the games with the player info.

Does anyone know were I can find a good quality one I can edit or can someone please make me one I can use every week. I can plug in the info myself I am just not that good to make one from scratch.
